Insurance Policy
Please be advised, Sharon DeJesus, NP, does not participate with any insurance plans. You are welcome to use your FLEX SPENDING or HEALTH SAVINGS ACCOUNT credit cards. She does not have a contract with plans, so clients pay on the day of visit.  If you have a private or PPO insurance plan, they may cover part of visit and the plan would reimburse clients directly. She is an out-of-network provider with the AETNA, CIGNA, United Health Care / United Behavioral Health, Blue Cross Blue Shield, MultiPlan, GHI, MuitiPlam, Humana and Pomco networks.
Clients with out-of-network benefits are offered the option of the office submitting claim information electronically on your behalf so that you may be reimbursed by your insurance company for the percentage of the out-of-network benefit. This means clients pay for the visit on the day of service and then AETNA, UBH/UHC, BCBS, MultiPlan or POMCO may reimburse you on a later date, depending on your insurance plan.

Payment Options
Cash and all major credit cards are accepted for payment, including flex spending and health spending credit cards. Checks are no longer accepted. Upon request, an invoice/statement will be provided for your records.

Please check your coverage carefully. Call your insurance company and ask the following questions:
    • Does my plan cover outpatient mental health services or behavioral health services?
    • Does my plan cover out-of-network providers? If so, how much?
    • Do I have a yearly deductible?
    • Is the yearly deductible a different amount if I use an out-of-network provider?
    • Once my deductible is met, what percentage of the amount paid will be reimbursed to me?
    • Do I need an authorization code?
    • What is the claims mailing address or what is the Payer ID?
    • How many visits are covered per year?

Cancellation Policy
Notify us at least 24 hours in advance if you are unable to attend your appointment so that we may offer that appointment to a client on our waiting list. If you miss your scheduled appointment , and you have not notified us at least 24 hours in advance, you will be required to pay a $75 fee.

Why is Sharon De Jesus, NP, an Out-of-Network Provider?
Insurance companies increasingly require more and more information from the private offices in order for the plans to pay for the services provided.  Many plans require prior authorization for visits, where the office must release a client's personal information, treatment plans, and more.  This led to hours spent on phones with insurance plans, excessive paperwork, lost revenues, and less time spent helping clients.  As a result, she has offered services as an out-of-network provider since 2012.
